The call came in at 11:47 p.m.

Anonymous tip. Luxury hotel. Guest in respiratory distress. I was already pulling on my coat before the dispatcher finished the address.

That's the thing about ER work — your body moves before your brain catches up. Five years of night shifts will do that. You stop asking why and just go.

The Meridian Hotel was twelve blocks from Seattle Grace Memorial. I knew the building. Alistair had taken me there for our third anniversary. Oysters and champagne on the forty-second floor. He'd tucked a strand of hair behind my ear and said I was the only thing in his life that made sense.

I thought about that in the elevator. Then I stopped thinking about it.

Suite 4108. The door was already open — a hotel manager hovering in the hallway, wringing his hands, face the color of old chalk. I heard the sound before I saw anything. That wet, dragging wheeze. A throat fighting itself.

I went in.

The bedroom. King bed, sheets twisted. A woman I didn't know — dark hair, red dress pooled on the floor — sitting upright against the headboard with her hands at her own throat, eyes wide and glassy with the particular terror of someone who cannot pull air through a closing passage.

And Alistair.

My husband. Standing at the foot of the bed in an open dress shirt, barefoot, staring at me with an expression I had never seen on his face before. Not guilt. Something more complicated than guilt. Something that looked, for just a second, like he was calculating the distance between where he stood and the nearest exit.

I looked at him for exactly as long as it took me to locate the woman's pulse point with two fingers.

Then I looked away and didn't look back.

'Anaphylaxis,' I said. To no one in particular. To the room. I unzipped my bag. 'How long has she been like this?'

The manager said something. I wasn't listening. My hands already had the epinephrine pen. I uncapped it, pressed it to her outer thigh, held it for ten seconds. Her body jerked. I counted her pulse. Watched her throat. Waited.

The wheeze began to loosen. Slowly. Then a little faster.

I instructed the manager to call 911 for transport and monitor her breathing. I told him the exact words to say when the paramedics arrived. I recapped my pen. I zipped my bag.

Alistair said my name.

I picked up my coat from the chair where I'd set it. I walked out.

The elevator was mirrored on three sides. I watched myself descend — forty-one floors of a woman in scrubs with a medical bag and a wedding ring and a face that had gone somewhere very quiet and very far away. I looked like someone performing the concept of calm. I looked like I was doing it well.

I drove home.

Our penthouse was dark. I didn't turn on the lights. I sat down at the kitchen island and put both hands flat on the cold marble and just — stayed there. The refrigerator hummed. Rain moved across the floor-to-ceiling windows in slow diagonals. Seattle at midnight looks like a city that's been crying for hours and hasn't decided to stop yet.

I sat there until 4 a.m. I know because I watched the clock on the microwave change.

At 8 a.m. sharp, I walked into my attorney's office and filed for divorce.

Alistair came home three days later.

I was in the kitchen making coffee. He walked in wearing the particular stillness he uses when he's decided something — that boardroom composure, shoulders back, jaw set, the silence before an announcement.

'Withdraw the filing,' he said.

I poured my coffee.

'Ariya.'

'I heard you.'

'This is going to damage the company. You know that. The optics of a public divorce filing —'

'I'm not withdrawing it.'

He was quiet for a moment. The refrigerator hummed. Outside, a ferry horn sounded once across the Sound.

'If you force my hand,' he said, 'you will regret it.'

I looked at him then. Really looked. He was still handsome — that had never been the problem. Dark eyes, the kind of face that photographs well and says nothing. I had loved that face so completely, once, that the memory of it felt like something that had happened to a different woman.

Maybe it had.

'Okay,' I said.

I picked up my coffee and my keys and my hospital badge. I went to work my next shift.

I didn't know, walking out that door, that Delaney Fisher was already moving. That she had already made phone calls, already identified a disgraced surgeon with a grudge and a willingness to use it. That the next phase of something I couldn't yet name was already in motion, quiet and precise, like a hand closing around a throat.

I just knew my patients needed me.

So I went.
