The doctor told me I might not survive the week.

I sat across from him in a private medical office owned by the Moretti Syndicate, my hands resting over the file stamped with the Ravenscar family crest. Outside the tinted windows, armed guards stood watch along the hospital corridors while luxury sedans came and went beneath the pouring rain. In our world, even hospitals belonged to someone.

“Your condition is deteriorating faster than expected, Mrs. Ravenscar,” Dr. Harris said carefully. “Dialysis may keep you stable for a little while longer, but if your body crashes again, there may be nothing we can do.”

I lowered my eyes to the report in my lap.

End-stage kidney failure.

The words blurred together after a while.

Dr. Harris hesitated before turning another page. His voice became quieter. “There’s something else you should know. Your condition worsened because you only have one kidney remaining.”

My fingers curled slightly against the paper.

That part, I already knew.

Damon didn’t.

Six years ago, Damon Ravenscar nearly died after his armored convoy was ambushed outside Palermo Harbor. Rival families opened fire on his vehicle after a failed weapons negotiation, and by the time his men dragged him out of the wreckage, he was already bleeding out. Organ failure followed two days later.

The Ravenscar family searched half the underworld for a compatible donor.

I was the match.

Damon was unconscious when I signed the consent papers. I still remembered waking up afterward with stitches burning through my side while Evelyn Ravenscar stood beside my hospital bed dressed in black silk and diamonds, looking more like a queen than a mother worried for her son.

“Do you want Damon’s love,” she asked me softly, “or do you want his gratitude?”

I was stupid enough to think there was a difference.

I looked up sharply.

The silence in the room stretched long enough for Dr. Harris to understand everything from my face alone.

He stopped there.

When I left the clinic, the city looked painfully normal. Black umbrellas moved along crowded sidewalks. Men in tailored suits smoked outside casinos owned by rival syndicates. Somewhere nearby, church bells rang through the rain while life carried on as though mine had not just been reduced to a countdown.

I returned to the Ravenscar estate shortly before sunset.

Two armored moving trucks stood outside the mansion gates.

For one ridiculous second, I thought Damon had remembered our anniversary.

Then I saw Lilith Ashenford stepping out of his car.

She wore a pale dress beneath a cream-colored coat, one hand resting protectively over the gentle curve of her stomach while Damon guided her toward the entrance with his hand against her back. The gesture was instinctive. Possessive.

The guards bowed their heads as they passed.

Servants hurried to carry her luggage inside.

My chest tightened slowly.

Damon noticed me standing beneath the front archway, but his expression never changed.

“You’re back,” he said calmly.

I looked at Lilith’s stomach.

She lowered her eyes with practiced shyness. “I hope this won’t upset you, Aveline. Damon said it would be safer if I stayed here for a while.”

Safer.

Inside my home.

Damon stepped closer, removing his leather gloves as though this conversation were ordinary. “Lilith’s pregnancy is unstable. The doctor advised complete rest until the first trimester passes.”

I waited for the rest.

His gaze shifted away for the briefest moment before he finally said, “You’ll move into one of the guest suites tonight. The master bedroom is closer to the private elevator and medical floor.”

The master bedroom.

The room I spent years waiting for him in while he handled cartel negotiations and midnight executions. The room where I learned how to sleep alone beside cold sheets and untouched pillows.

Now another woman would sleep there because she carried his heir.

Lilith stepped toward me gently, almost apologetic. “Please don’t misunderstand. I never wanted to take anything from you.”

That was when I noticed the sapphire bracelet around her wrist.

My grandmother’s bracelet.

The only thing I brought with me when I married into the Ravenscar family.

Damon followed my gaze and frowned slightly. “It’s just jewelry, Aveline.”

The command came naturally to him.

As though it made perfect sense for his wife to kneel at another woman’s feet.

I crouched down silently and gathered the papers.

Ultrasound scans.

Hormone reports.

Medication schedules.

Then I saw the case number printed across the bottom page.

The air disappeared from my lungs.

Three years ago, after my miscarriage, the fertility clinic had given me a small recovery file before discharging me. I remembered staring at the case code for hours because it was the only thing left connected to the child I lost.

Lilith snatched the paper from my hands too quickly.

Her smile trembled for half a second before settling back into place. “Thank you,” she said softly. “Pregnancy makes me careless these days.”

Damon noticed nothing.

He was too busy watching her.
