I turned thirty-six in OR 3, elbow-deep in someone else's emergency.

The baby came at 3 AM—cord wrapped twice, mother hemorrhaging, the kind of delivery that doesn't ask your permission. After that, two urgent consults, a full surgical roster, and fourteen hours of chart notes written on coffee fumes and muscle memory. By the time I capped my pen and leaned back in my chair, the clock on the wall read 6:47 PM.

Thirty hours. Not one message from Dalton.

I told myself he was in meetings. I'd been telling myself that for months.

Dr. Chen stopped by my office doorway on his way out, coat half-on, keys in hand. He looked at me the way people look at someone they feel sorry for but don't know how to say it.

"Happy birthday, Sara."

"Thanks, Kevin." I smiled. He left. The smile dissolved the second he turned the corner.

I sat there for a moment, listening to the ward settle into its evening rhythm—the soft beeps, the squeak of sneakers on linoleum, the low murmur of the night shift taking over. I love this place. That's the honest truth. Even exhausted, even hollow, there's a part of me that feels most like myself inside these walls.

I was pulling on my coat when I heard her.

Loud doesn't cover it. She was a detonation.

"Where is she? I want the doctor who's been ignoring me all week!"

Janelle Rivera was maybe eight months along, moving through the ward like she owned it, one hand on her belly and the other jabbing the air. I recognized the name from the charts—routine prenatal follow-up, no complications flagged. I stepped out of my office.

"Ms. Rivera. I'm Dr. Spencer. How can I—"

"You." She stopped. Looked me up and down like I was something she'd found on the bottom of her shoe. "You're the one who's been giving me the runaround. You think I don't know negligence when I see it?"

The ward went quiet. I felt every pair of eyes on us—nurses, residents, the orderly by the supply cart. Twenty-something people watching.

"Ms. Rivera, let's step somewhere private and—"

"I'm not going anywhere." Her voice climbed higher. "You people think you can treat patients like garbage and nobody says anything? I've been waiting three hours. Three hours! My blood pressure is through the roof and nobody cares because you're too busy playing doctor—"

"I understand you're frustrated," I said. My voice came out steady. Years of training. "I'd like to review your chart and address every concern. But I need you to lower your voice so we can—"

She spat in my face.

Direct. Deliberate. Full stop.

The ward didn't just go quiet. It stopped breathing.

I didn't move. I couldn't. There was saliva on my cheek and a roaring in my ears and thirty hours of exhaustion pressing down on my chest like a physical weight. Nurse Chloe Park was at my side in seconds, a hand on Janelle's arm, another nurse already calling security. I reached up and wiped my face with the back of my hand.

I was still standing there when Dalton walked in.

I felt something loosen in my chest when I saw him. Even then. Even after everything, some stupid, loyal part of me thought: he came.

He didn't look at me. He looked at Janelle Rivera.

And then he crossed the ward, took my arm—not gently—and leaned down close to my ear.

"Apologize to her," he said. Quiet. Firm. Like I was a problem he was managing.

"Dalton—"

"Sara." His grip tightened. "She's a patient. Just apologize."

I looked at him. I looked at the woman who had just spat on me. I looked at my nurses, my residents, Chloe with her eyes wide and her mouth pressed into a thin, horrified line.

And I bowed my head.

"I'm sorry for any distress you experienced today, Ms. Rivera."

The words came out clean and precise. Like I was reading from a script. Like I was someone else entirely.

Dalton let go of my arm.

Later, alone in our apartment—Dalton at another work dinner, his third this week—I poured a glass of water I didn't drink and sat down at the kitchen table with my phone.

I don't know what made me do it. Some quiet instinct. Something that had been whispering for months and finally decided to speak.

Jazmine Rivera. I typed the name into the search bar.

The profile loaded. Public. Curated. Beautiful.

I scrolled slowly.

A restaurant I'd suggested to Dalton last spring—he'd said he didn't like the neighborhood. His hand on her waist at a rooftop event he'd told me was men-only. A comment thread, months long, full of inside jokes I'd never heard. A photo of two coffee cups, captioned *mornings like this*, posted on a Tuesday when Dalton said he had an 8 AM call.

I sat in the dark for a long time.

My thumb moved across my fingertips. Back and forth. Back and forth.
